Post-secondary planning for students with disabilities includes an academic, social, and emotional assessment of the student’s needs to be successful. Upon obtaining this pertinent data an appropriate and meaningful transition plan is developed. Participants will understand how the students’ academic achievement along with the current functional skills lead to an effective transition plan.
